Eligibility
Eligible older people with disability are those who:
- are currently accepted by the Commonwealth Disability Support for Older Australians (DSOA) program for ongoing services, or
- are previous clients of Disability SA who have transitioned to My Aged Care but have an arrangement in place with the DHS Equipment Program.

If you are eligible for equipment or home modifications through the Equipment Program, you will require an approved prescriber to assist you with an assessment and prescription.
Approved prescribers can be funded through:
- Disability Support for Older Australians (DSOA) - please speak with your DSOA Service Coordinator,
- My Aged Care Commonwealth Home Support Program or Home Care Package,
- SA Health or Country Health Connect,
- a Medicare Chronic Disease Management Plan through a GP, or
- a private therapist (health insurance or self-funded).
